How To Enroll
- Complete and sign the Application for SmartChoice Benefits and the Premium Authorization Agreement.
- Have your employees complete and sign the Employee Enrolment Form
- Enclose both a cheque marked “VOID” and a cheque for the first month’s premium including applicable provincial sales tax.
- Premiums will be automatically withdrawn from your business bank account each month.
- The required forms are available under Enrolment Forms
- Please note that your benefits and premium withdrawals will commence on the first day of the following month after we receive the original signed forms.
- Your claims kit including a booklet and prescription drug card for each employee will arrive during the first week of the month that your benefits are effective.
