How To Enroll

  • Complete and sign the Standalone C.I. Application and the Premium Authorization Agreement.
  • Complete and sign the Enrolment Form
  • Complete the C.I. Statement of Health Questionnaire
  • Enclose both a cheque marked “VOID” and a cheque for the first month’s premium including applicable provincial sales tax.
  • Premiums will be automatically withdrawn from your business bank account each month.
  • Please note that your benefits and premium withdrawals will commence on the first day of the following month after we receive the original signed forms.
